These worksheets are tools designed for
determining hospice eligibility within the Unipolicy guidelines that took
effect on December 1, 2007.
These documents replace the Hospice Quick Guide and will be reviewed by
Medicare's Region I Hospice Advisory Committee in the coming months. They
are guidelines and must be used appropriately in the clinical context.
Please send us your suggested revisions as you work with these documents.
