Hospice RNs

The Concord Regional Visiting Nurse Association (CRVNA) is seeking Hospice RN’s to care for patients with end of life care needs. The RN works with the multi-disciplinary team to provide skilled nursing care according to the patient’s plan of care.

 

Qualifications:
The qualified candidate will have a RN diploma and Associate Degree (BSN preferred); current NH nursing license; recent acute care experience skills; a minimum of two years nursing experience. Previous Hospice and/or oncology experience preferred.

SALARIED FULL TIME MANAGER

Salaried, full time Manager is responsible for overall management of a hospice and palliative care program.  With goals of quality patient care, a family supportive environment, appropriate resource utilization and financial soundness, the manager coordinates all aspects of hospice program.  Position provides a high level of visibility within the agency and local community. 

Position requires daily collaboration with internal and community resources, supervision of an interdisciplinary team, strong knowledge of hospice policies and regulations, strong leadership and team building skills and  knowledge of hospice financial parameters. 

Qualifications include valid NH RN license, a minimum of a Bachelors Degree, sophisticated business and management skills and minimum three years of hospice RN experience. Prefer minimum two years hospice management experience.  Successful applicants will be proficient in computer technology, self directed, a strategic thinker with exceptional problem solving skills and a personal commitment to the hospice concept.

Home Health & Hospice Care (HHHC) is a free standing, not-for-profit visiting nurse association. We have earned a superb reputation serving the communities of southern NH and northern MA for over 120 years. We are proud of our history as one of the first hospice programs in the state and offer superior care in patient’s homes, nursing facilities, and at our Community Hospice House in Merrimack.

HHHC is a premiere hospice provider in the community, offering patients a strong multidisciplinary team of caregivers, including: a Board Certified Palliative Care Physician, hospice certified nurses and aides, social work staff, chaplain, bereavement professionals, and trained volunteers.

We are currently recruiting for these important staff positions:

advanced registered nurse practitioner

The ARNP will work in collaboration with our Medical Director, who is Board Certified in Palliative Medicine, and our hospice interdisciplinary team in the delivery of palliative care to patients. The practitioner will perform initial patient evaluations, provide pain and symptom management, and also participate in the clinical education of our nursing team and community. Qualified candidates will have their ARNP in New Hampshire; Masters Degree is preferred. Three to five years palliative care experience in a hospice setting is desirable. Candidates should have proven skills in pain and symptom management, as well as experience working with an interdisciplinary team. A current NH nursing license, CPR certification, and controlled substance license are all required.

Hospice RNs

Part and full time case management positions are available with some weekend and on-call commitment.  You will provide skilled nursing intervention and supportive end-of-life care for patients in the home and nursing homes. Experience with hospice patients and interdisciplinary teams are highly desirable.

Community Hospice House RN (11p-7a)

This opportunity is full-time however can be a part-time job share position. Schedule is typically weekdays but will include weekend rotations and holidays.  Hospice experience is preferred.

DIRECTOR OF HOSPICE

Visiting Nurse Association of Manchester and Southern New Hampshire is seeking a qualified individual to provide leadership and direction to the Hospice Program. This individual will be responsible for the overall operations of the Hospice Program, as well as the strategic plan generating increased market share.

This individual will be responsible for the strategic direction, managing operations, educating the community and promoting clinical excellence of the Hospice Program. VNA of Manchester and Southern New Hampshire serves more than 5,000 individuals a year in over 22 towns in the Southern New Hampshire area.

Qualifications:
BS Degree in Nursing, Master's Degree preferred. Minimum of five (5) years experience in the hospice field, in addition to seven (7) years of progressive management responsibilities in health care programs. NH Licensure required

Rockingham VNA & Hospice is a not for profit, CHAP accredited organization providing home care and hospice services for the adult population in the Rockingham County.

REGISTERED NURSES

Full-time Monday – Friday;  Part-time, every Saturday/Sunday  or every other Saturday/Sunday and Monday    

Responsibilities:

  • Completing a comprehensive assessment including physical, psycho/social, and spiritual aspects of patient/family;

  • Analyzing the assessment data in determining nursing diagnoses;

  • Identifying expected outcomes to patient/family;

  • Developing a plan of care with interventions to obtain expected outcomes that evolve as patient needs change;

  • Implementing the interventions identified in the plan of care in a safe, timely manner, and according to agency standards.

Nursing and rehab staff are connected through the use of a laptop and the electronic medical record system.  Patient documentation and assessments are completed right on the computer. The RVNA offers the opportunity for staff to have one-on-one patient contact and a team approach while allowing staff to work independently out of the home.

Requirements:
Graduate of accredited RN program; 1 to 3 years experience; RN license-NH state or other compact state; valid driver's license; BCLS certified or within 30 days of hire.

Advance Registered Nurse Practitioner

Founded by volunteers in 1978, Seacoast Hospice is New Hampshire’s only non-profit, independent,  comprehensive-care hospice.  Serving residents in  Rockingham and Strafford Counties, Seacoast Hospice  has offices in  Exeter, Dover and Portsmouth.
