Home Health & Hospice Liaison – 40 Hours/Full Time

The Elliot Health System is looking for an individual to build, establish, and maintain strong referral relationships with physicians and physician practices, skilled nursing and assisted living facilities, hospital and discharge case managers, and the community.  The role of the Home Health & Hospice Liaison is to ensure all patients who would require and benefit from VNA services have access to those services. This individual will promote and educate about the programs and services the Visiting Nurse Association of Southern New Hampshire offers.

Required Knowledge and Skills

·  Ability to uncover and pursue business opportunities that are aligned with long term business goals

·  Must possess strong analytical, strategic and creative problem solving skills

·  Ability to execute sales plan, use professional selling skills and have territory management strategies necessary to build the     
    service lines of the Visiting Nurse Association.

·  Ability to develop and maintain strong business relationships with identified referral sources in service area.

Education: Education in Sales and Marketing or equivalent experience required. Graduate of an accredited school of nursing; BSN or equivalent preferred. CHPN preferred
Experience: 2 years experience in sales and marketing required with proven track record. Experience preparing and providing education programs preferred. Three to five years experience working in Hospice and Homecare preferred.
Certification/Licensure: Current Licensure in State of New Hampshire as a professional nurse required. CHPN preferred but not required. Valid driver’s license.
Software/Hardware: Familiar with MS Outlook, Work, Excel and/or Access. Familiar with setting up and maintaining a data base of contacts and activities.

APRN or Physician – Hospice Programs (Part to Full-Time 1st Shift + On-Call Rotation)

We are looking for a colleague - either 20 hours to full time hours per week - physician/ nurse practitioner to help us in our growing needs to assist with pain, and symptom management as a continuation of both our hospice programs (home, nursing home & hospice house) as well as our palliative care program.

A physician colleague will be responsible for a team of hospice patients, rotate through our hospice house as well as being part of our growing palliative care team. This colleague, if an APRN, will primarily work closely with our Hospice and Palliative Board certified physicians to care for our patients at our hospice house but may also be responsible for making other visits, evaluating patients and attending weekly interdisciplinary team meetings. The physician or APRN must be licensed in NH and have end-of-life experience. The APRN will be included in the on-call rotation, which includes weeknights and about one weekend per month.

RN - HOSPICE HOUSE

                              
- Per Diem (All shifts) Position includes weekend, holiday, on-call and off-shift rotations as   
  needed by the program.
                                 
Responsibilities include working with patients at our Hospice House with end of life care needs. Qualifications: Licensed by the State of New Hampshire as an RN; Minimum of 2-years recent acute care experience skills required. Previous Hospice and/or oncology experience preferred. Is able to understand and carry out directions and instructions, record messages and keep records. Is able to read, write and speak English. Is physically and emotionally capable of performing the job requirements.

REGISTERED NURSE - HOSPICE

- Full time - Monday thru Friday with weekend rotation/on-call
- Days

Reporting to the Hospice Program Manager; provides professional nursing care as assigned. Coordinates care planning with other disciplines. Services are provided to hospice patients residing in the Rockingham County; must be willing to travel.

Requirements: Graduate of accredited RN program. 1 - 3 years hospice experience; IV and chemotherapy experience helpful; RN License - N.H. State or other compact state; valid drivers license. BCLS certified or within 30 days of hire.
