RN - HOSPICE COMMUNITY
CRVNA is seeking an experienced RN to provide
care to Hospice patients in the community. The RN works with the
multi-disciplinary team to provide end of life skilled nursing care according to
the patient’s plan of care.
Qualifications: Licensed by the State of New Hampshire as an RN; a minimum of
two years nursing experience with recent acute care experience skills is
required. Previous Hospice and/or oncology experience preferred.
Work schedule: Flexible full-time (37.50) hours per week including a weekend and
on-call rotation responsibilities.

working with patients at our Hospice House with end of life care needs.

Nurse
Practitioner - Hospice
This position will oversee the care of Hospice patients/families for the VNAH of
VT & NH. Responsibilities include, but are not limited to: Collaboration with
the interdisciplinary hospice team in the development of care; Consultation with
and education of care providers; Research and development of best practices
related to improving palliative outcomes, effectiveness and safety of services;
Provides assessment visits, recertifying patients for hospice care, and
participate in bridging our home care patients to hospice. Qualifications:
· Advanced practice degree from accredited school of nursing
· RN license eligible in VT and NH
· Valid driver’s license and agency required auto insurance
· Apply the principles, methods and techniques of medicine to provide ongoing
patient care.
· Legal regulations, procedures to administer patient care
· Knowledge of medical equipment and instruments
· Prepare, maintain records
· Generate and analyze reports

Executive Director (Registered Nurse)
St. Joseph Home & Hospice
Care – A Division of St. Joseph Healthcare

The Executive Director reports directly to the Director of
Operations and is responsible for the effective administration of St. Joseph
Home and Hospice Care’s (SJHHC) programs in accordance with mission, by-laws,
policies, procedures, and ethical standards of St. Joseph Hospital (SJH), the
Joint Commission and all local, state and federal regulatory requirements. This
position will collaborate with the SJH administration, SJHHC Board of Directors,
committees and staff to enhance SJHHC operations and to evaluate programs and
plan for the future. He/she is also responsible for providing an environment
that supports continued professional growth and open communication as well as
formulating and recommending goals, objectives, policies, plans and programs for
administrative and Board review.
This position will demonstrate the philosophy, mission, and core values of St.
Joseph Hospital (SJH) in performance of position responsibilities and will do so
with a focus on the values of excellence, collaboration, stewardship, and
respect. Participate in system-wide activities, committees and teams as required
to carry out responsibilities for representation of SHHC. Does so as a team
player, with a focus on the greater need of SJH, and integrates hospital and
system-wide team concepts into areas supervised.
Responsibilities will also include, assuring compliance with regulatory and
accreditation requirements of the state, JCAHO and other agencies, as
appropriate as well as oversight of the Operational Policy manual for SHHC
assuring it is always kept current. Will assure effective communication
throughout areas supervised. Communicates Home Care and Hospice Department
goals, and integrates St. Joseph Home & Hospice Care and system-wide goals into
the process. .
The Executive Director’s leadership style should demonstrate organizational
commitment, customer service orientation, team orientation, conceptual and
analytical thinking, tolerance for ambiguity, and self-management ability that
is flexible and biased toward change management and achievement of objectives.
He/she will be available for interpersonal communication with staff and ensures
the necessary information and progress reports are channeled back to the
Director of Operations satisfying the need for timely and accurate information.
Direct daily business operations and implement financial policies accounting
system and cost controls, formulate budgets for review and approval by the
finance committee and review financial statements, operating results and
performance evaluations on an ongoing basis.
Position will establish and maintain, in elaboration with SJH marketing
department, an ongoing public relations program and business plan to increase
community understanding, improve relations with physicians and other health care
clinicians and increase revenues. Negotiate contracts and agreements for
provision of services and assure enforcement of those agreements. Implement
record and statistical reporting systems and program documentation, planning and
evaluation. Provide leadership in the development of Organization objectives,
standards of care and policies and procedures affecting the programs of
organization.
A Nursing Degree required (Bachelor’s or higher preferred); Master’s degree in
business, public administration or other related field preferred. 5 to 6 years
of proven experience in Home Health Care Management is essential. Incumbent will
be self-directed and require little oversight in performance of duties, which
include financial, human resource, technical, quality management, and team
building concepts. This position also requires excellent interpersonal skills,
analytical and conceptual thinking ability, and team building ability, with an
emphasis on performance and achievement. The ability to affect change
management, and commitment to customer focused management of services as well as
a Community involvement background necessary.

Required Knowledge and Skills

and maintaining a data base of contacts and activities.

APRN or Physician – Hospice Programs (Part to Full-Time 1st Shift + On-Call
Rotation)
We are looking for a colleague - either 20
hours to full time hours per week - physician/ nurse practitioner to help us in
our growing needs to assist with pain, and symptom management as a continuation
of both our hospice programs (home, nursing home & hospice house) as well as our
palliative care program.
A physician colleague will be responsible for a team of hospice patients, rotate
through our hospice house as well as being part of our growing palliative care
team. This colleague, if an APRN, will primarily work closely with our Hospice
and Palliative Board certified physicians to care for our patients at our
hospice house but may also be responsible for making other visits, evaluating
patients and attending weekly interdisciplinary team meetings. The physician or
APRN must be licensed in NH and have end-of-life experience. The APRN will be
included in the on-call rotation, which includes weeknights and about one
weekend per month.

provided to hospice patients residing in the Rockingham County; must be willing

Graduate of accredited RN program. 1 - 3 years hospice experience; IV and
chemotherapy experience helpful; RN License - N.H. State or other compact state;
valid drivers license. BCLS certified or within 30 days of hire.
